Kaiser / SEIU Local 49 Education Fund Application Process and Instructions
Application Process:
Members
submit requests for funds to Human Resources, c/o Local 49 Education
Committee KPB-13. All requests are reviewed by the Human Resources
Adviser to the Education Committee. Requests are then forwarded to the
Treasurer, who reviews them for pre-approval. If the Treasurer has
concerns about the request, s/he will bring it to the Education
Committee for review and a final decision.
The Education Committee is made up of four SEIU Local 49 members who work at Kaiser, plus an Administrative adviser.
If
your request is approved, the Treasurer will send you a letter of
approval. Your application is sent to Human Resources for a signature
and then to Accounts Payable to issue the funds. The process takes
approximately 2 to 4 weeks.
If denied, the committee will review your application and send you a letter explaining why the request was denied.
Appeals
to any decision of the Local 49 Education Committee must be submitted
in writing to the Medical Labor Management Steering Committee at Human
Resources, KPB-13. Questions for committee members may be submitted to
the Local 49 Education Committee at Human Resources KPB-13.
Application Instructions:
Obtain a current Local 49 Education Expense Request form (available on the Internal Kaiser website).
Send completed request form to Local 49 Education Committee, c/o Human Resources, KPB-13, within 45 days of course completion.
When
requesting funds for a seminar or workshop, attach two copies of the
registration form and a brochure describing the event (must include
date and cost of the event).
If requesting reimbursement, enclose proof of payment: either a copy of your canceled check (both sides) or a copy of receipt(s).
When requesting funds for college reimbursement, please enclose the following:
a. Kaiser Permanente tuition reimbursement denial forms pertaining to this request.
b. A copy of your canceled check (both sides) or a copy of receipt as proof of your payment.
c. Copies of receipts for lab fees, book fees, and any other fees that apply.

Submit
time off request if needed to Manager / Supervisor. If time off
request is denied, forward the Education Request Form along with the
denied Time Off Request to Local 49 Education Committee, c/o Human
Resources, KPB-13. An employee whose time off request for education was
denied will be given priority for a subsequent event.

Request
will be processed by the 2nd Wednesday of each month, and notification
will be sent to employees within 14 days of the committee meeting.
