• You qualify for benefits after working 1040 hours (six months full time)
•
You must work (or be on paid vacation) for 120 hours each month (30
hours per week) to qualify for benefits the following month.

Short Term Disability | William C. Earhart Company• You qualify for benefits after working 1040 hours (six months full time)
• You must work (or be on paid leave) for 120 hours each month (30 hours per week) to qualify for benefits the following month
• You are covered if you have to miss work due to sickness, pregnancy or an off-the-job injury
Benefits begin…
• after one week if you miss work due to illness or pregnancy
• immediately if you are hospitalized
• immediately if you are injured
• Disability payments are $150/week for length of disability (up to 26 weeks)
•
Two exceptions: no payments for car accidents (covered by car
insurance) or on-the-job injuries (covered by workers’ compensation)

Dental | William C. Earhart Company• You qualify for benefits after working 1040 hours (six months full time)
• You must work (or be on paid leave) for 120 hours each month (30 hours per week) to qualify for benefits the following month.
• Accepted by almost all dentists in Portland
• Covers 80% of standard procedures
• Covers 50% of surgeries or special procedures
• Maximum benefit is $1000 per year

Death and Dismemberment | William C. Earhart Company • You qualify for benefits after working 1040 hours (six months full time)
• You must work (or be on paid leave) for 120 hours each month (30 hours per week) to qualify for benefits the following month.
• $8,000 if you die in an accident
• $4,000 if you die due to a sickness, or lose both hands, or both feet, or one hand and one foot, or lose sight in both eyes
• $2,000 if you lose one foot, one hand, or the sight in one eye
• $1,000 if your child (between 6 months and 19 years) dies
• There are some exceptions, such as if you cut off your hand on purpose or if someone commits suicide
